Report an accident or incident

Sentinel online accident and incident reporting system
All accidents, incidents, near misses and cases of work-related ill health must be reported as soon as possible via the University’s Sentinel system.
This includes accidents and incidents that occur both on University premises and during University-organised activities held off University premises (ie fieldwork).
Any accident involving serious injury should immediately be reported to your Health and Safety Manager.
If you are not a registered Sentinel user
Contact your Health and Safety Co-ordinator or Health and Safety Manager with details of what happened at the earliest opportunity. If you are unsure who your local representative is, please use the staff directories below:
Health and Safety Co-ordinators: seach by department to find your local representative
Health and Safety Managers: listed by faculty/area of responsibility
