Report an accident or incident

Please note: this information relates to the recording of accidents and incidents. Guidance for summoning First Aid and/or the emergency services can be found on our Calling an Ambulance to campus webpage.

Sentinel online accident and incident reporting system

All accidents, incidents, near misses and cases of work-related ill health must be reported as soon as possible via the University’s Sentinel system.

This includes accidents and incidents that occur both on University premises and during University-organised activities held off University premises (ie fieldwork).

Any accident involving serious injury should immediately be reported to your Health and Safety Manager.

If you are not a registered Sentinel user

Contact your Health and Safety Co-ordinator or Health and Safety Manager with details of what happened at the earliest opportunity. If you are unsure who your local representative is, please use the staff directories below:

Health and Safety Co-ordinators: seach by department to find your local representative

Health and Safety Officers: listed by faculty/service

Health and Safety Managers: listed by faculty/area of responsibility

Reporting arrangements may vary across local areas for those without Sentinel access. If you need further support, please contact Health and Safety Services at  or call 0113 3434201
